Annual Sports Physicals Night, June 4, 2007 |
On Monday, June 4th, GHS' will
host its annual Sports Physicals Night for all students planning to participant
in sports. This includes students from Churchill, Costa, Lombard and Galesburg
High School.
Although this exam does not meet state requirement for incoming
freshmen, it can be used for athletics. Copies of the sports physicals need to
be on file in BOTH the school nurses office and the athletic department prior to
any sports participation at the High School.
If the students use the “school
required” physical for sports, by I.H.S.A. rules, it will remain current (good
for 365 days from date of exam). If the physical expires before the end of a
season, notification will be given by the Athletic Department and an updated
physical would need to be submitted. (A spring season 9th grade
athlete, whose freshman physical was done before June of their 8th
grade year is encouraged to attend the All Sports Physical Night.)
Parents must
complete and sign the athlete’s health history information prior to the exam.
There will be no exemptions. Physical forms will be available at each school
prior to the exam. The $5.00 donation will be collected at the time of the
exam. The forms will be stamped once they are approved and the donation has
been received.
